Section 1
Eligibility
1.1
Open to students currently enrolled in grades 9–12 or the equivalent secondary-school level in their home country.
1.2
Candidates must be 20 years of age or younger at the time of the IBMEO international round.
1.3
Candidates must be a citizen or permanent resident of the country they register to represent. Where a candidate holds both statuses in different countries, they may choose which country to represent, but that choice is final for the competition year.
1.4
There is no cap on how many students from a single school or country may register for the USBMEO qualifying exam — only advancement to IBMEO is limited by country ranking (see Section 5).
1.5
The competition is administered entirely in English. No translated exam versions are offered.
Section 2
Exam Format & Rules
2.1 Structure
2.1.1
The USBMEO qualifying exam consists of a single section: 55 multiple-choice questions, each with four answer choices.
2.1.2
Questions draw from the following core subject areas. Not every area is weighted equally, and the exact split changes year to year.
Biomechanics
Biomaterials
Bioinstrumentation
Physiological systems
2.1.3
There is no penalty for an incorrect answer. Blank answers receive no credit, so candidates are encouraged to attempt every question.
2.1.4
The IBMEO international round follows the same 55-question, single-section format, calibrated to a higher difficulty than the USBMEO qualifying exam.
2.2 Testing Window
2.2.1
The USBMEO exam is open for a 12-hour window on October 10, 2026, from 12:00 PM to 11:59 PM Eastern Time, so candidates in any time zone can find a reasonable start time.
2.2.2
Once a candidate opens the exam, a visible timer begins and counts down for the full session length without pausing.
2.2.3
The exam closes automatically when the timer reaches zero, and no further answers may be entered after that point or after the candidate presses Submit.
2.3 Proctoring & Technical Issues
2.3.1
All USBMEO and IBMEO sessions run under remote proctoring software. Candidates need a working webcam and a stable internet connection to sit the exam.
2.3.2
If a candidate's connection drops mid-exam, the clock keeps running. Reconnecting returns the candidate to the exam with whatever time remains — no extension is granted for connectivity lost on the candidate's end.
No re-sits. The Olympiad does not offer a second attempt for technical issues that occur on a candidate's own device or network.
2.3.3
The Olympiad organizers may invalidate a score if the proctoring record shows behavior inconsistent with independent, unaided work.
Section 3
Registration
3.1 How to Register
3.1.1
Registration is completed online through the official form at ibmeo.org/#register, followed by payment through the site's secure checkout.
3.1.2
The form collects the information below, which becomes the candidate's record of identity for the competition:
a.Student full name and email
b.School name and grade
c.Country the student is representing
d.Parent or guardian name, email, and phone number
3.1.3
Every registrant must check the honor-code box confirming they will not use unauthorized materials or share exam content, and that they understand the exam is proctored.
3.1.4
It is the candidate's responsibility to have their parent or guardian's consent before registering, and to enter accurate information — inaccurate entries may result in disqualification at the organizers' discretion.
3.2 Fees & Refunds
3.2.1
Registration for the USBMEO qualifying exam is $25, covering the exam platform, proctoring software, and question development.
3.2.2
There is no separate fee to compete at the IBMEO international round — qualification is included in the original USBMEO registration.
All fees are final. Once payment is confirmed, registration fees are non-refundable, including for candidates who do not sit the exam. Confirm your eligibility before paying, or write to support@ibmeo.org first.
3.3 Confirmation & Errors
3.3.1
A confirmation record is sent to the student's email once payment succeeds. If nothing arrives within 24 hours, contact support@ibmeo.org before the registration deadline.
3.3.2
Registrations completed outside the official form — for example, through a third party — are not recognized and may be declined.
3.3.3
The Olympiad is not responsible for registrations that fail to process because of issues with a candidate's own device, browser, or connection.
Section 4
Academic Integrity
4.1
Candidates must complete the exam independently. Notes, textbooks, calculators beyond what the platform provides, and outside assistance of any kind are prohibited unless explicitly stated in the exam instructions.
4.2
Sharing exam questions, screenshots, or answers — before, during, or after the testing window — is a violation of the honor code and may result in disqualification for every student involved.
4.3
Impersonation, or sitting the exam on behalf of another registrant, results in immediate disqualification of both parties and forfeiture of any award.
4.4
The organizers may request identifying documentation at any stage to confirm a candidate's identity, age, or country of residence.
Rescindable awards. Medals, honors, or IBMEO invitations obtained through a violation of this section may be rescinded after the fact, and the student may be barred from future Olympiad cycles.
Section 5
Advancing to IBMEO
5.1
Advancement from USBMEO to the IBMEO international round is based purely on score, ranked separately within each participating country.
The top 5 scorers per country on the USBMEO qualifying exam receive an invitation to compete at IBMEO on October 24, 2026.
5.2
Invited students must confirm attendance and, if requested, provide proof of age and citizenship or permanent residency before the IBMEO round. Failure to provide documentation forfeits the invitation.
5.3
Attending IBMEO is optional. A student who declines passes their invitation to the next-highest-ranked eligible student from the same country.
5.4
Countries with fewer than 5 eligible registrants send all of their eligible scorers, and no invitation is transferred internationally between countries.
Section 6
Scoring & Awards
6.1
Every registrant who completes the USBMEO exam receives an official digital certificate of participation, regardless of score.
6.2
USBMEO distinctions — Gold, Silver, Bronze, and Honorable Mention — are awarded to top performers, with at least 10% of all participants receiving one of these four honors.
6.3
At the IBMEO international round, Gold medals go to at least 20% of participants, or 10 students, whichever is fewer, with additional Silver, Bronze, and Honorable Mention distinctions awarded below that line.
